Most low-FODMAP food lists are just two long columns: eat this, avoid that. They’re useful for a quick check at the grocery store, but they don’t explain why a food is on the "avoid" side, which means every new food you meet outside the list is a guess. Once you know which of the five FODMAP subtypes a food actually contains, the list stops being a memorization exercise and starts being a set of rules you can apply on your own.

What FODMAP actually stands for, in practical terms
FODMAP is short for Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols. In plain terms, these are five families of short-chain carbohydrates that are poorly absorbed in the small intestine. Instead of being absorbed, they travel on to the large intestine, where they draw in water and get fermented by gut bacteria — producing the gas, bloating, cramping, and altered bowel habits that define an IBS flare. The diet doesn’t restrict "bad" foods; it restricts these specific carbohydrate structures, which is why unrelated foods like garlic and apples can both be problems while structurally similar foods like garlic-infused oil and grapes are fine.
Fructans: the wheat, garlic, and onion group
Fructans are the most common trigger in a typical Western diet, largely because they’re baked into staple foods most people eat daily. Wheat, barley, and rye are high in fructans, which is why regular bread, pasta, and most breakfast cereals are on the restrict list — not because of gluten, but because of the fructan content. Garlic and onion are the other major fructan sources, along with leeks (the white part specifically), artichokes, and asparagus.
Low-FODMAP swaps in this category are usually about substitution rather than elimination: rice, quinoa, oats, gluten-free bread, and corn-based products (polenta, tortillas, popcorn) replace the grains, while garlic-infused oil and the green tops of spring onions or leeks replace the alliums — the fructans don’t dissolve into oil, so you keep the flavor without the trigger.
Lactose: the dairy group
Lactose is a disaccharide, and the issue is enzymatic rather than structural: people with reduced lactase activity can’t fully break it down, so it ferments instead of absorbing. Cow’s milk, regular yogurt, ricotta, cottage cheese, and cream cheese are the higher-lactose dairy foods.
This is the easiest category to substitute in, because lactose-free versions of milk, yogurt, and soft cheeses exist and taste identical — the lactase enzyme is simply added during processing. Hard, aged cheeses (cheddar, parmesan, Swiss, brie) are naturally low in lactose regardless of the lactose-free label, since most of the lactose is removed during the cheese-making process.
Excess fructose and polyols: the fruit group
Fruit is where the food list gets confusing, because different fruits are restricted for entirely different reasons — and sometimes both at once. Excess fructose (fructose in a food exceeds the glucose content, so the surplus isn’t absorbed efficiently) is the issue in apples, pears, mango, and watermelon. Polyols — sugar alcohols like sorbitol and mannitol — are a separate trigger found in stone fruits like cherries and plums, and in some apple and pear varieties as well, which is why those two fruits show up on both lists.
Lower-FODMAP fruit choices — oranges, strawberries, blueberries, kiwi, and pineapple — tend to have a more balanced fructose-to-glucose ratio and minimal polyol content, which is the actual reason they’re better tolerated, not because they’re "less sugary."
GOS: the legume group
Galacto-oligosaccharides (GOS) are the primary trigger in legumes — lentils, chickpeas, kidney beans, and baked beans. Humans lack the enzyme to break the bonds in GOS chains at all, low-FODMAP or not, which is why legumes are restricted by portion size during the elimination phase rather than avoided outright; a small serving may be tolerated where a full cup isn’t.

Why the trigger-type framing matters more than the list itself
Printed food lists go out of date the moment you eat somewhere that isn’t on them. A restaurant dish, a packaged snack, a friend’s home cooking — none of it comes labeled "fructan" or "polyol." Once the categories click, though, you can make a reasonable call in the moment: a creamy sauce is probably a dairy question, a stir-fry with garlic and onion is probably a fructan question, a big fruit salad is probably a fructose-and-polyol question. That’s the actual skill the low-FODMAP diet is teaching, and it’s the reason a working clinician builds a food list this way instead of handing over two undifferentiated columns.
